The 47th Medical Operations Squadron Family Health Clinic is your medical home. The FHC provides primary care to active duty, dependents and retirees. The FHC consists of two family health teams. FHTs are comprised of a physician, nurse practitioner or physician assistant, a nurse and five medical technicians. Each FHT has a goal empanelment of approximately 4,400 patients assigned. The FHT mission is to provide holistic, efficient, evidence-based medical care to meet the individual needs of each patient, thus embracing the concept of the patient centered medical home.
PCMH focuses on the patient being the center of healthcare and the driver of care rather than a passive recipient. PCMH provides care that is truly patient-centered, considers patients' cultural traditions, personal preferences and values, family situations and lifestyles. It makes the patient and their loved ones an integral part of the care team who collaborate with healthcare professionals in making clinical decisions.
Patient centered care puts responsibility for important aspects of self-care and monitoring in the patient's hands, along with the tools and support they need to carry out that responsibility. Patient-centered care ensures transitions between providers and healthcare settings are respectful, coordinated and efficient.

Active duty acute concerns: Active duty members with acute, new onset illness may call central appointments at (830) 298-6333, to book an acute appointment with a family health provider or family health team from 7:30 a.m. to 4:30 p.m. Monday through Thursday and 8:30 a.m to 4:30 p.m. Friday's. If no acute appointment is available, a telephone consult to the FHT nurse will be created and a call back will be made to the member for telephone triage, home care options, referral to a local ER or urgent care or booking of a later available appointment.
**There is no sick call for FHC**. To make an appointment to the FHC, call (830) 298-6333. You can also call (830) 703-6199, to reach the FHP on call after normal business hours for home care instructions.
Patients requesting profile extensions, convalescent leave or separations or retirement paperwork completion may drop off requests and supporting documents completely filled out at FHD. The FHT will review the request and process the paperwork as indicated within 72 hours. Profile extensions and updates take five to seven business days to process.
